Get Yourself A Disabled Parking Permit In Hawaii

You can exercise for disability parking placards or licence plates provided you conformed the disability criteria established by the country of Hawaii Disability and Indication Access Board. The primary factor is a limited ability to walk. You need a physician to certify that your disability is qualified under the law.


Instructions


1. Provide the application to your licensed practicing physician who certifies that your disability is qualified under the law. The certification also designates the duration of the disability as either long-term (expected to last 4 years or more), or as temporary which is expected to last up to 6 months.4.


Complete the information required of the applicant on Side 1 of the form. Select the type of parking permits being requested, either one or two placards, or disability license plates.


3. Ascertain that you meet the disability criteria specified in the state of Hawaii rules and regulations. Note that certain conditions such as blindness, deafness, and mental disability do not qualify as disabilities for receiving disability placards and plates.2. Download the Person With A Disability Parking Permit Application from the state of Hawaii Disability and Communication Access Board website.


Take the completed and certified application form to the proper county agency as listed on the form. Only renewal applications or applications on which a physician certifies that the applicant cannot physically appear in person can be mailed. Bring proof of identity when appearing at the county agency.
